Living Choices

What is Living Choices?

Living Choices is an assisted living program that helps you live independently in an assisted living facility, allowing you to enjoy your daily activities and maintain your health. It is an excellent alternative to institutional care.

For example, if you need assistance with bathing, toileting, eating, or drinking, trained staff at the assisted living residence are available to help. The program also offers social activities, medication assistance, and many other services — all designed with one goal: to keep you living in the community.

How do I apply?

Call your local DHS county office or the Choices in Living Resource Center at 1-866-801-3435 to explore your options.
